4. Form 5 - Declaration as to Inventorship:

This form is used to declare the inventors of the patented invention, including their names, addresses, and contributions to the invention.

5. Form 18 - Request for Examination:

Applicants must use Form 18 to request the examination of their patent application. It initiates the examination process by the Indian Patent Office.

6. Form 26 - Representation Opposing Grant of Patent:

Form 26 is used by third parties to oppose the grant of a patent. It includes details of the grounds for opposition and is submitted to the Controller of Patents.

7. Form 27 - Statement Regarding the Working of Patented Inventions on a Commercial Scale in India:

Patentees and licensees must submit Form 27 annually to provide information about the commercial working of the patented invention in India. This helps ensure that patents are being used for public benefit.

8. Form 28 - Statement Regarding the Working of Patented Inventions on a Commercial Scale in India:

Form 28 is used to submit a statement regarding the working of patented inventions by government agencies or institutions.

9. Form 29 - Request for Leave to Amend a Complete Specification Before Grant:

This form is used to request permission to make amendments to a complete specification before the grant of a patent.

10. Form 30 - Request for Leave to Amend a Complete Specification After Grant:

 After the grant of a patent, Form 30 is used to request permission to make amendments to the complete specification.

 

These forms are crucial for maintaining transparency, adhering to procedural requirements, and facilitating communication between inventors, applicants, and the Indian Patent Office.
